Notice of Intent (NOI)

For Applicants filing a Notice of Intent to do work in a resource area and/or buffer zone.

  1. The proposed work is within the Buffer Zone as defined in 310 CMR 10.04 (if the filing of a Notice of Intent is required) or within Land Subject to Flooding, as defined in 310 CMR 10.57(2), or within the Riverfront Area, as defined in 310 CMR 10.58; and
  2. The proposed work will disturb less than 1,000 square feet of surface area within the Buffer Zone and/or Land Subject to Flooding or less than 1,000 square feet of Riverfront Area, provided the work conforms to the requirements of 310 CMR 10.58(4)(c)2.a. pertaining to analysis of alternatives; and
  3. Neither a Department of Army permit nor a Chapter 91 Waterways license from the Department is required.

Extension Permit

Applicants requesting an Extension Permit (to extend the date of the expiration of an active Order of Conditions) do not need a special form. The Applicant shall submit a written request for an Extension Permit at least 30 days prior to the expiration of the Order of Conditions. The request must include the DEP file number, the date of expiration of the Order of Conditions, the period of time for which the extension is sought (i.e., one additional year), the name, address and telephone number of the person making the request, and the reason the extension of time is needed in order to complete the project. The request shall be accompanied by a $50 fee in the form of a check made payable to the City of Woburn.

Certificate of Compliance

Upon completion of a project approved under an Order of Conditions, applicants seeking a Certificate of Compliance shall submit seven hard copies plus an original of the Request for a Certificate of Compliance form along with the other items referenced in the final condition of the Order of Conditions to the Conservation Commission
